1. Medical History: Adults 成人病历采集

Doctor’s Questions: If the answer to question No.1 is No, go to No. 5.

1）Do you have any existing medical health problems?

你现在有什么已经确诊的病？

2）Are you on any medication for existing health problem?

3）How often do you have to take in?
